Dental Plans
The Guardian dental plan includes a network of preferred dentists. If you receive treatment from preferred dentists, you will receive enhanced benefits. However, you also have the option of receiving treatment from the dentist of your choice who is not a member of the network. Benefits for treatment from non-preferred dentists will be paid at the lower reimbursement level and may be subject to usual, customary and reasonable benefit limitations.
Dental Coverage Effective Date
Coverage begins on the first day of the month following the date of hire.
Dental Employee Cost
The Corporation pays 100% of the premium for employees and all eligible benefits.
Dental Plan Changes
There is no "open enrollment" for dental benefits. If you or your dependents do not enroll when you first become eligible, benefits are drastically reduced upon subsequent enrollment. Only preventive benefits are paid for the first 6 months. Basic services are covered after 6 months. Major services are not covered until you have been insured under the plan for 12 consecutive months. All newly acquired dependents (spouse, newborn, adopted child, etc.) must be added to the dental plan within 31 days of eligibility (date of marriage, birth, adoption, etc.).

Vision Plans
The Corporation pays 100% of the premium for employees and all eligible benefits. Enrollment is not automatic. The plan provides the following benefits:
- Eye examination - once every 12 months ($10 Copay)
- Standard lenses*/or contact lenses - once every 24 months

Standard frames every 24 months
- Coverage available every 12 months if there is a change in prescription (Refer to summary of Benefits located in UHR for complete details)
Employees may use a participating or non-participating provider. However, if you use a non-participating provider, your benefits will be paid at a reduced rate. Participating provider directories are available in the Campus Service Center. Refer to the plan booklet for co-pay and maximum allowance details.
You don't need an ID card nor a claim form. Just contact your VSP doctor and tell them you area vsp member.
Vision Coverage Effective Date
The effective date is generally the first of the month following the employee's start date.
